Welcome to St. Gerard School and thank you for inquiring!  All prospective families should complete the enrollment paperwork at https://stgerardlima-oh.finalforms.com/

Once you have complete the enrollment paperwork, please contact Mrs. Marta Truex by stopping by the school or calling the office at 419-222-0431 to complete the next steps of the enrollment process.

When enrolling, all new families are asked to bring or upload the following documents for their child or children: 

• birth certificate
• baptismal certificate (if applicable)
• previous school records and report cards
• proof of custody (if applicable) 
• proof of residence.
